Michael Baptista, M.D. has had extensive training in areas such as general surgery, thoracic, robotic, minimally invasive, and bariatric surgery. He now leads his practice where he specializes in weight loss surgery and general surgery. He understands the multifactorial problem of obesity and the surgical options for it.

Dr. Baptista is a consultant and proctor in bariatric surgery, teaching colleagues that are interested in the field of weight loss surgery. He is a published author in the field and has given many courses, both on how to perform bariatric surgery and how to follow up with patients afterwards.

David N. Csikai, MD

Dr. Smith founded First Coast Dermatology in 1979. He completed his undergraduate education at Mount St. Mary’s College, where he graduated magna cum laude, and then attended medical school at the Ohio State University College of Medicine, where he was elected to the Alpha Omega Alpha Honor Medical Society. Dr. Smith completed residencies in dermatology and pediatrics at Yale New Haven Hospital and Duke University Hospital.

Dr. Smith is a Clinical Associate Professor at the University of Florida College of Medicine-Jacksonville, where he has volunteered at the Pediatric Dermatology Clinic for over 25 years. He also serves in the Dermatology Clinic at Volunteers in Medicine-Jacksonville.

Dr. Smith is board certified in Dermatology and Pediatrics. He is a member of the American Academy of Dermatology, Florida Medical Society, Duval County Medical Society, and is past president of the Jacksonville Dermatology Society. He is also a member of the Rotary Club of Deerwood. When not working, Dr. Smith enjoys hiking and biking.

Dr. Rosario began his education with a pre-med major at the University of the Sacred Heart and then attended the Ponce School of Medicine in Puerto Rico, where he received an academic distinction award. He completed an internship in Internal Medicine at Damas Hospital and residencies in Damas Hospital and the National Capital Consortium in Washington, D.C.

Dr. Rosario was active duty in the military from 1996-2006 and served as a Commander in the United States Navy, and as the department head of both Internal Medicine and Dermatology.

He received multiple commendations and Military awards including Army, Navy Commendation Medals, and a Humanitarian Service Medal. Dr. Rosario is listed in Who’s Who Among Medical Professionals and was the President of the Jacksonville Dermatology Society in 2009.
Dr. Rosario is a Fellow of the American Academy of Dermatology and the American Society for Mohs Surgery. He is Board Certified in Dermatology and Internal Medicine. He is also a member of the American Academy of Dermatology, and the American Society of Mohs Surgery, the Florida Society of Dermatologic Surgery, the Jacksonville Dermatology Society and the Duval County Medical Society. Sanjay Swami, MD

Dr. Swami began practicing in Jacksonville in the fall of 2003 and founded First Coast Allergy and Asthma in 2005. He was born and raised in South Carolina. He attended the University of South Carolina and obtained his M.D. from the Medical University of South Carolina. Dr. Swami completed his residency training in internal medicine at Wake Forest University/Baptist Medical Center. He moved to Jacksonville in 2003 after completing his fellowship training in allergy and immunology at Johns Hopkins Allergy and Asthma Center. Dr. Swami is board certified in allergy and immunology by the American Board of Allergy and Immunology, a conjoint board of the American Board of Internal Medicine and American Board of Pediatrics.

Dr. Swami is on the board of the local chapter of the American Lung Association. He has also appeared on local television on several occasions to discuss allergic conditions. Amit Chokshi, MD
